Clinic Case Manager, 50/50 position

Full-time Clinic Case Manager for community-based care management limited within Franklin County. Team assignment and pre-established caseload to collaboratively plan and monitor services provided to mentally ill individuals.  This position is comprised of 50% clinic tasks and 50% case management duties.  Provide a minimum of 624 patient service hours annually (52/month).

The standard work week is Monday-Friday, 8:00 a.m. - 5:00 p.m.  Hourly rate of pay is $20 hourly with mileage reimbursement applicable.

KEY FUNCTIONS:

A Case Manager acts as a crucial link between the client, the healthcare system and the community working to ensure that the client receives comprehensive and integrated care to support behavioral and health needs to attain goals that support independent living. The primary objective of a Case Manager is to provide support and coordination of care for individuals with behavioral health conditions that also incorporate assistance with the development of care plans, coordination of services, advocacy, monitoring and follow-up, intervention, collaboration, empowerment and documentation of billed services.

  • Clinic tasks are comprised of general reception duties such as greeting clients at the lobby desk, answering in person and telephone inquiries and scheduling appointments.

QUALIFICATIONS: 

  • Bachelor's level degree required;
  • Must be cempathetic with the desire to provide aid and make an impact within the lives of persons served;
  • Initiative, organizational skills and resiliancy are necessary;
  • Prior exposure or experience to mental illness or prior case management experience preferred;
  • Must complete 90-day paid training program;
  • Provide a minimum of 624 patient service hours annually (52/month);
  • Valid Ohio driver's license, automobile and good driving record are required; and
  • Pre-employment criminal background check and drug testing are mandatory
