Behavioral Health Case Manager

We are seeking a dedicated behavioral health Case Manager to join our dynamic team focusing on delivery of exceptional community-based care within Franklin County. As an integral treatment team member, you will manage a designated caseload and engage in collaborative planning and oversight of services for individuals with mental health conditions.

This full-time position operates from Monday-Friday, 8:00 a.m.-4:30 p.m.  We offer a competitive hourly rate of $25.00 with additional mileage compensation at the standard Federal rate for client-related transportion during appointments or referrals.  Provide a minimum of 1,248 patient service hours annually (104/month).

KEY Responsibilities:

A Case Manager acts as a crucial link between the client, the healthcare system and the community working to ensure that the client receives comprehensive and integrated care to support behavioral and health needs to attain goals that support independent living. The primary objective of a Case Manager is to provide support and coordination of care for individuals with behavioral health conditions that also incorporate:

  • Assistance with the Development of Care Plans:  Collaborating with the client, their family, healthcare providers and other relevant stakeholders to create personalized care plans that address the specific needs and goals of the individual based upon an assessment and evaluation of the client to better understand health needs, health disorders or comorbidities, substance abuse issues, treatment and recovery as well as other related barriers in order to promote success to attain goals for independent living.
  • Coordination of Services:  Coordinating and facilitating access to a range of behavioral health services, including therapy, counseling, psychiatry, substance abuse treatment and recovery, obtaining benefits, housing assistance, employment support and more.
  • Advocacy:  Advocating for the client's rights and needs within the healthcare system, legal system or social service agencies to ensure they receive appropriate care and support with dignity and respect.
  • Monitoring and Follow-Up:  Tracking the client's progress, monitoring treatment adherence and collaborating with other treatment professionals to adjust the care plan as needed to ensure meaningful progress towards goals.
  • Crisis Intervention:  Providing immediate assistance and support during crises or emergencies to ensure the safety and well-being of the client.
  • Education and Support:  Offering education to the client and their family about behavioral health conditions, treatment options, coping strategies and available resources.
  • Collaboration and Communication:  Coordinating communication and collaboration among various stakeholders, including healthcare professionals, social workers, legal representatives, benefit providers and others involved in the client's network of care.
  • Documentation and Reporting:  Maintaining accurate and up-to-date records of the client's progress, interactions, services provided and other relevant information within the Electronic Medical Record (EMR) system as required by North Central Mental Health Services, Inc. to comply with Managed Care Organizations, State of Ohio and CARF standards.
  • Empowerment:  Empowering the client to take an active role in their own treatment and recovery by providing them with the necessary tools, information and support.
  • Cultural Sensitivity:  Demonstrating cultural competence and sensitivity in working with diverse populations to ensure that services provided are appropriate and respectful of the client's cultural background and beliefs.

Qualifications: 

  • Bachelor's level degree required;
  • Must be caring, compassionate and have a strong desire to provide aid and make an impact within the lives of persons served;
  • Motivated, organized and detailed personal traits to advocate and coordinate services effectively on behalf of patients;
    • Prior knowledge of community resources helpful;
    • Prior exposure or experience to mental illness or prior case management experience preferred;
  • Provide support, education and communication to patients about their behavioral health issues;
    • May involve clients, families or legal entities, etc.
  • Must complete 90-day paid training program;
  • Provide a minimum of 1,248 patient service hours annually (104/month);
  • Valid Ohio driver's license, automobile and good driving record are required; and
  • Pre-employment criminal background check and drug testing are mandatory
