Position Overview:We’re seeking a full-time Licensed Physical Therapist to provide direct care in home settings. This role includes assessment, treatment planning, implementation, and educating patients and families to support recovery and independence.
What You'll Do:
Transform Lives: Conduct patient evaluations and create individualized treatment plans per physician orders.
Empower Recovery: Deliver therapy including exercises, mobility training, and assistive device instruction.
Educate and Support: Teach patients and families treatment procedures and home exercises.
Drive Progress: Reassess progress and adjust plans to improve outcomes.
Collaborate: Work with physicians and care teams for seamless treatment.
Optimize Environments: Assess homes and recommend modifications or adaptive tools.
Maintain Excellence: Complete timely documentation in Epic EMR.
Qualifications
Bachelor’s Degree in Physical Therapy (required).
Active Illinois PT license in good standing.
Valid Driver’s License.
Passion for home health encouraged—training available!
Schedule:Monday–Friday (8:00 AM–4:30 PM) with occasional weekend/holiday rotation. 4-10s available.
Rotating weekend schedule (every 6th weekend).
Compensation & Benefits
Base salary: $65,000–$110,000/year (based on experience).
$2,500 sign-on bonus (paid in 2nd pay period).
Mileage reimbursement at IRS rate.

Work Location:Field-based role with territory: East St. Louis, Cahokia Heights, Dupo, & Washington Park.
Productivity:27.5 productivity points per week (avg. 5.5/day).
