Why You'll Appreciate This Position
Salary: $80,000–$100,000 annually
$3,500 sign-on incentive
Monday–Friday, 8:00 AM to 5:00 PM; occasional weekends
No on-call duties
Mileage reimbursement at $0.625/mile
Typically 5–6 patient visits/day
Weekly productivity goal: 25 visits
Digital charting via Home Care Home Base
Collaborative team focused on patient care and balanced schedules

Responsibilities
Deliver in-home OT services per physician orders
Perform evaluations, create care plans, complete OASIS assessments
Provide interventions: ADL/IADL/MRADL training, energy conservation, adaptive equipment, fine motor skills, sensory rehab, and cognitive therapy
Educate patients and families on treatment plans and safety techniques
Collaborate with interdisciplinary teams and support discharge planning
Supervise Certified Occupational Therapy Assistants (COTAs) and support staff as needed
Requirements
Active OT license in Colorado
Minimum 1 year of OT experience (home health, SNF, rehab, or hospital accepted)
Current CPR certification
Valid driver’s license and appropriate auto insurance

My grandfather grew up a farm boy in rural West Texas, never venturing beyond the county lines until age 19. His life soon took a remarkable path: he served courageously as a bombardier in World War II and later as a jet fighter pilot during the Korean War. After his military service, he returned to his roots—his cherished farm—where he found peace in the familiar rhythms of the land he loved.
In his later years, it was compassionate home health and hospice professionals who made it possible for him to remain at home, surrounded by comfort, dignity, and family. Their care made all the difference.
