Behavioral Health Case Manager

Join our dedicated team as a full-time Behavioral Health Case Manager in the supportive community of Franklin County. Engage directly with a dynamic team to strategically coordinate and monitor services for individuals with mental health challenges.

Our standard work schedule is Monday through Friday from 8:00 AM to 4:30 PM. This position offers a competitive hourly wage of $25, plus mileage reimbursement for travel related to client support activities. You'll be required to document your services in our electronic medical records system, achieving at least 1,248 billable hours per year.

CORE RESPONSIBILITIES:
The role involves being an essential liaison between clients, healthcare systems, and community resources while ensuring comprehensive care that fosters independent living. Responsibilities include:

Developing Individualized Care Plans: Partnering with clients and their networks to form tailored care plans based on thorough assessments.

Service Coordination: Connecting clients to necessary behavioral health services such as therapy, substance abuse treatment programs, benefits coordination or employment aid.
Advocacy Efforts: Standing up for client rights within various systems ensuring they receive the respect and appropriate care they deserve.
Progress Monitoring & Adjustments: Evaluating client progress regularly and adjusting treatment strategies accordingly.
Crisis Response Support: Offering prompt support during urgent situations guaranteeing client safety.
Educational Outreach & Supportive Counseling: Informing clients about their conditions while providing effective coping strategies along with resource guidance.
Interprofessional Collaboration & Communication: Fostering teamwork among diverse healthcare providers involved in patient care networks.
Meticulous Documentation: Keeping accurate records aligned with organizational standards as well as regulatory requirements.
Empowering Clients: Encouraging self-management by equipping them with tools which facilitate active participation in recovery processes.
Cultural Competency: Delivering respectful service attuned to varied cultural backgrounds ensuring inclusivity.

REQUIREMENTS:
A Bachelor's degree is mandatory;
Genuine compassion toward others;
Self-motivated personality with organizational skills;
Community resource knowledge is advantageous;
Experience or familiarity with mental health challenges or case management preferred but not required;
Compulsory completion of our paid 90-day training program;
Billing productivity must meet or exceed 104 hours/monthly requirement; Valid Ohio driver's license along with reliable transportation and clean driving record essential; Mandatory pre-hire background checks including criminal history review and drug screening adherence.
