Care Navigator

We're looking for a compassionate, organized, and resourceful care navigator to support individuals in accessing health, wellness, and social support services. You'll work remotely to guide clients through care options, help coordinate services, and ensure they receive the support they need.

This is a people-first role ideal for someone who is empathetic, detail-oriented, and experienced in client-facing or case management positions.

Key Responsibilities:

Serve as the main point of contact for clients seeking care or support

Help clients understand and access available resources (medical, behavioral, financial, etc.)

Coordinate appointments, referrals, follow-ups, and service navigation

Maintain accurate documentation of client interactions and status updates

Communicate with care providers, case managers, and support teams

Follow up with clients to ensure needs are met and barriers are addressed

What We're Looking For:

U.S. citizenship required

2+ years of experience in care coordination, case management, social services, or customer support

Strong interpersonal and communication skills, both written and verbal

Empathetic, solution-focused, and able to work with clients from diverse backgrounds

Organized and tech-savvy; comfortable using systems like Salesforce, EHRs, or CRM tools

Ability to work independently in a remote environment and manage multiple clients.
