Travel Physical Therapist (PT) – Home Health Glendale (High Desert Territory – Hesperia/Victorville), California Contract Duration: 13 Weeks (March 23, 2026 – June 22, 2026) Estimated Weekly Pay: $2,161.80 Schedule: Day Shift | 5x8 Hours (8:00 AM – 5:00 PM) | 40 Hours Guaranteed* Position Overview Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health setting based out of Glendale, CA, covering the High Desert territory (Hesperia/Victorville). This is a field-based role where the Physical Therapist will provide in-home rehabilitation services focused on improving mobility, restoring independence, and enhancing patient outcomes through individualized, evidence-based care. The therapist will work independently while maintaining communication with the care team. Facility Information Facility Type: Home Health Agency Work Setting: Field-based (patient homes) Coverage Area: High Desert (Hesperia, Victorville, surrounding areas) Clinical Coverage Areas Therapists will provide care for: Post-surgical recovery Chronic condition management Mobility and balance impairments Neurological and orthopedic rehabilitation Home safety and functional independence training Key Responsibilities Perform in-home patient evaluations and functional assessments Develop individualized treatment plans based on patient needs Deliver therapeutic exercises, mobility training, and rehabilitation interventions Complete OASIS documentation accurately and timely Monitor patient progress and adjust care plans accordingly Educate patients and caregivers on home exercise programs and safety Coordinate care with physicians and interdisciplinary teams Maintain compliance with all documentation and regulatory standards Manage daily visit schedules efficiently Required Qualifications Minimum 1 year of Home Health experience Active California Physical Therapist License (or proof of application at submission) OASIS experience required (flexible if strong HH experience) BLS Certification (AHA) Valid driver's license and reliable personal transportation Strong communication and documentation skills Preferred Qualifications Previous travel experience Experience managing independent caseloads Familiarity with home health workflows Scheduling & Assignment Details 40 hours guaranteed based on completed visits Productivity: 25 visits/week = 40 hours Monday – Friday typical schedule (varies by territory) Mileage reimbursement provided (IRS standard rate) On-call rotation may be required (discuss during interview) Important Notes Local candidates accepted at travel rate CA license preferred in hand at submission (pending accepted if already applied) Coverage territory may impact pay rate Strong time management and independence required Compliance Requirements Candidates must complete all onboarding and compliance requirements including: Home Health skills checklist Requested Time Off (RTO) submitted at time of application Timely documentation compliance Orientation modules (billable and completed prior to assignment) Vaccination Requirements: COVID-19 Vaccine (Medical/Religious Exemptions Allowed) Flu Vaccine (Medical/Religious Exemptions Allowed) Why Consider This Assignment Competitive weekly compensation Flexible field-based work environment Mileage reimbursement provided Opportunity to work across diverse patient populations High-demand home health experience for future roles
