Travel Physical Therapist (PT) – Home Health Valley City, North Dakota Contract Duration: 13 Weeks (March 4, 2026 – June 3, 2026) Estimated Weekly Pay: $2,176.90 Schedule: Day Shift | 5x8 Hours (8:00 AM – 5:00 PM) | 40 Hours Guaranteed Position Overview Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health setting in Valley City, ND. This is a field-based role where the Physical Therapist will provide in-home rehabilitation services to patients recovering from surgeries, injuries, and other medical conditions. The focus is on improving mobility, restoring independence, and enhancing overall patient outcomes through personalized, evidence-based care. The therapist will work independently while maintaining communication with the broader healthcare team. Facility Information Facility Type: Home Health / Hospice Agency Work Setting: Field-based (patient homes) Coverage Area: Up to 60-mile radius from Valley City, ND Charting System: HomeCare HomeBase Clinical Coverage Areas Therapists will provide care for: Post-surgical recovery Fall-related injuries Mobility and functional impairments Home health and hospice patients Strength, balance, and endurance training Key Responsibilities Perform in-home patient evaluations and functional assessments Develop individualized treatment plans based on patient needs Deliver therapeutic exercises, mobility training, and rehabilitation interventions Manage a caseload of approximately 5–7 patients per day Monitor patient progress and adjust treatment plans accordingly Educate patients and caregivers on home exercise programs and safety Maintain accurate and timely clinical documentation Coordinate care with interdisciplinary healthcare teams Manage daily travel and scheduling efficiently Required Qualifications Minimum 1 year of Home Health Physical Therapist experience Active North Dakota Physical Therapist License BLS Certification Valid driver's license and auto insurance Strong communication and documentation skills Preferred Qualifications Previous travel experience Experience with HomeCare HomeBase EMR Ability to work independently in field-based settings Scheduling & Assignment Details 40 hours guaranteed per week Monday – Friday schedule Shift: 8:00 AM – 5:00 PM No weekend requirements No holiday requirements Important Notes Coverage area includes up to a 60-mile radius No clinical/manager interview required Candidates must not be employed with CommonSpirit/CHI/Dignity facilities within the past 12 months (radius rule applies) Requested Time Off (RTO) must be submitted upfront (no additional RTO accepted after submission) Facility may cancel up to 4 shifts during the 13-week assignment Compliance Requirements Candidates must complete all onboarding and compliance requirements including: Active state licensure verification BLS certification Skills checklist and competency validation Orientation (3–5 shifts) Completion of all required documentation and onboarding modules Why Consider This Assignment Competitive weekly compensation Stable weekday schedule (no weekends or holidays) Independent, field-based work environment Opportunity to work with diverse home health and hospice patients Strong demand experience for future home health roles
