Travel Physical Therapist (PT) – Acute Care / Outpatient Hybrid Alamosa, Colorado Contract Duration: 13 Weeks (March 9, 2026 – June 8, 2026) Estimated Weekly Pay: $2,316.30 Schedule: Day Shift | 5x8 Hours (8:00 AM – 5:00 PM) | 40 Hours Guaranteed Position Overview Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a hybrid acute care and outpatient role in Alamosa, CO. This position primarily focuses on outpatient therapy services with partial inpatient responsibilities, including evaluation coverage. The Physical Therapist will provide individualized, evidence-based care to improve mobility, restore function, and support patient recovery across a diverse patient population. The therapist will work in a collaborative healthcare environment and may support nearby facilities as needed. Facility Information Facility Type: Acute Care Hospital Total Beds: 49 Work Setting: Hybrid (Outpatient + Inpatient support) Charting Systems: OBix, McKesson, Pyxis, SBAR Parking: Free Clinical Coverage Areas Therapists will provide care for: Outpatient rehabilitation (primary focus) Inpatient evaluations (limited coverage) Orthopedic and post-surgical patients Neurological and mobility impairments Geriatric and general medical populations Key Responsibilities Perform patient evaluations and functional mobility assessments Develop individualized treatment plans Deliver therapeutic exercises, mobility training, and rehabilitation interventions Provide outpatient therapy services and occasional inpatient evaluations Monitor patient progress and adjust treatment plans accordingly Maintain accurate and compliant clinical documentation Collaborate with interdisciplinary healthcare teams Educate patients and families on recovery and home exercise programs Required Qualifications Active Colorado Physical Therapist License (or Compact License) Minimum 2 years of experience (1 year may be considered) BLS Certification Strong clinical and documentation skills Preferred Qualifications ACLS Certification Experience in both outpatient and acute care settings Familiarity with multiple EMR systems Scheduling & Assignment Details 40 hours guaranteed per week Monday – Friday schedule Shift: 8:00 AM – 5:00 PM Weekend Requirement: Approximately once every 2 months (inpatient coverage) Floating required between nearby facilities (within ~15 miles) Important Notes Role is primarily outpatient with partial inpatient responsibilities May float to nearby facility (Conejos – ~15 minutes away) Start date must allow ~2 weeks for compliance processing Locals accepted License must be in hand at submission RTO Policy: Must be submitted upfront No RTO within first 2 weeks Maximum 10 days total No full-week RTO allowed Only one submission allowed per candidate Compliance Requirements Candidates must complete all onboarding and compliance requirements including: Active state licensure verification BLS certification (ACLS preferred) Two supervisor references (within last 18 months) Skills checklist and competency validation Completion of onboarding and orientation Why Consider This Assignment Competitive weekly compensation Unique hybrid outpatient + inpatient experience Work in a smaller, collaborative hospital setting Exposure to diverse patient populations Opportunity to build versatile clinical experience
