Travel Physical Therapist (PT) – Home Health Berkeley, California Contract Duration: 13 Weeks (March 23, 2026 – June 22, 2026) Estimated Weekly Pay: $2,311.18 Schedule: Day Shift | 8:00 AM – 5:00 PM | 40 Hours Per Week Position Overview Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health assignment in Berkeley, CA. This is a field-based role where the therapist will provide in-home rehabilitation services, focusing on improving patient mobility, independence, and overall quality of life. The therapist will work closely with patients in their homes, delivering personalized care and treatment plans. Key Responsibilities Perform comprehensive patient evaluations and reassessments in the home setting Develop and implement individualized treatment plans Provide therapeutic exercises and functional mobility training Utilize manual therapy techniques and rehabilitation modalities Educate patients and caregivers on home exercise programs and safety practices Maintain accurate and timely clinical documentation Monitor patient progress and adjust care plans as needed Care Setting Home Health (Field-based) Patient visits completed in residential settings Productivity based on completed visits (25 visits = 40 hours guaranteed) Required Qualifications Minimum 1 year of Home Health experience OASIS experience required (preferred but may consider strong HH background) Active California Physical Therapist License (or pending with proof of application) BLS Certification (AHA) Valid driver's license and reliable transportation Strong documentation and communication skills Preferred Qualifications Previous travel experience Experience managing independent caseloads in home health Assignment Details Schedule Options: Tuesday–Saturday or Wednesday–Sunday 40 hours guaranteed based on productivity Mileage reimbursement (IRS standard) On-call rotation may be required (discuss during interview) Modules completed during orientation (billable) Important Notes Local candidates accepted at travel rate Candidates must submit RTO at time of application Home Health checklist required Must have applied for CA license prior to submission if not in hand Why Consider This Assignment Competitive weekly compensation Flexible scheduling options Opportunity to work independently in a home health setting Mileage reimbursement for travel between patients Supportive and structured onboarding process
