Travel Physical Therapist (PT) – Home Health Stockton / Central Valley, California Contract Duration: 13 Weeks (March 23, 2026 – June 22, 2026) Estimated Weekly Pay: $2,352.59 Schedule: Day Shift | 5x8 Hours (8:00 AM – 5:00 PM) Possible Work Schedules: Tuesday – Saturday Wednesday – Sunday Coverage Territory: Central Valley / Stockton Area Position Overview Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health field position in the Central Valley / Stockton region. The Physical Therapist will provide in-home rehabilitation services, helping patients regain mobility, improve strength, and maintain independence following illness, injury, or surgery. This role requires strong clinical judgment, time management, and the ability to work independently while coordinating care with interdisciplinary teams. Workload Expectations 25 completed visits per week = 40 hours Hours are guaranteed once 25 visits are completed Mileage between patient homes reimbursed at IRS standard rate Key Responsibilities Conduct patient evaluations in the home setting Develop individualized treatment plans Provide therapeutic exercises and mobility training Monitor patient progress and perform reassessments Educate patients and caregivers on home exercise programs Maintain accurate and timely documentation Collaborate with physicians and care teams Required Qualifications Active California Physical Therapist License (or pending with proof of application) BLS Certification (AHA) Valid driver's license and reliable transportation Preferred Qualifications Home Health experience preferred OASIS documentation experience preferred Strong time management and communication skills Compliance Requirements Flu Vaccine Required (Medical or Religious Exemptions Allowed) COVID Vaccine Required (Medical or Religious Exemptions Only) Additional Requirements RTO must be submitted at the time of application On-call rotation may be required (discussed during interview) Home Health checklist completion required Additional Assignment Details Locals accepted at travel rate Travel pairs accepted but assigned to separate territories with individual transportation Orientation modules are billable Why Consider This Assignment Competitive weekly compensation Flexible home health assignment in Central California Opportunity to work independently in a field-based setting Mileage reimbursement between patient visits
