Travel Physical Therapist (PT) – Home Health Hayward / Fremont, California Facility: Neogen Care Contract Duration: 13 Weeks (March 23, 2026 – June 22, 2026) Estimated Weekly Pay: $2,311.18 Schedule: Day Shift | 5x8 Hours (8:00 AM – 5:00 PM) Possible Work Schedules: Tuesday – Saturday Wednesday – Sunday Coverage Territory: Hayward & Fremont, California Position Overview Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health field position with Neogen Care in Northern California. The Physical Therapist will provide in-home rehabilitation services , helping patients improve mobility, restore function, and maintain independence following illness, injury, or surgery. The therapist will work independently while collaborating with physicians, nurses, and care teams to deliver high-quality patient-centered care. This role is ideal for therapists who enjoy working in a home health environment with flexible field-based patient care . Workload Expectations 25 completed visits per week = 40 hours Hours are guaranteed once 25 visits are completed Mileage between patient homes reimbursed at IRS standard mileage rate Key Responsibilities Conduct comprehensive patient evaluations in the home setting Develop individualized treatment plans and rehabilitation goals Provide therapeutic exercises and mobility training Educate patients and caregivers on home exercise programs and safety practices Monitor patient progress and adjust treatment plans as needed Maintain accurate and timely documentation in the EMR system Collaborate with physicians and interdisciplinary care teams Required Qualifications Active California Physical Therapist License (or pending license with proof of application) BLS Certification (AHA) Valid driver's license and reliable personal transportation Preferred Qualifications Home Health experience preferred OASIS documentation experience preferred Strong time management and independent clinical skills Compliance Requirements Flu Vaccine Required (Medical or Religious Exemptions Allowed) COVID Vaccine Required (Medical or Religious Exemptions Only) Additional Requirements Requested Time Off (RTO) must be submitted during application On-call rotation may be required (discussed during interview) Home Health checklist completion required before start Additional Assignment Details Locals accepted at travel rate Travel pairs accepted but assigned to separate territories with their own transportation Orientation modules are billable and completed during onboarding Why Consider This Assignment Competitive weekly compensation Flexible home health travel assignment Opportunity to work independently while supporting patient recovery Mileage reimbursement between patient visits
