Travel Physical Therapist (PT) Glendale / Palmdale / Lancaster, California Home Health 13-Week Assignment Day Shift | Flexible Schedule | 40 Hours Pay Package Estimated Weekly Pay: $2,400/week Position Overview Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health assignment covering Palmdale and Lancaster, CA . This role focuses on delivering one-on-one care in a home health setting , helping patients improve mobility, function, and independence in their own environment. Key Responsibilities Evaluate and treat patients in a home health setting Develop and implement individualized treatment plans Perform functional mobility training and therapeutic interventions Complete OASIS documentation and maintain accurate records Educate patients and families on care plans and home exercises Coordinate care with interdisciplinary teams Requirements Active California PT License (or pending at submission) BLS Certification (AHA) Minimum 1 year of Home Health experience OASIS experience preferred (required or strong HH experience considered) Valid driver's license and reliable transportation Assignment Details Schedule options: Tues–Sat or Wed–Sun 25 visits = 40 hours guaranteed Mileage reimbursed (IRS standard) Locals accepted at travel rate Coverage area: Palmdale / Lancaster On-call rotation may be required
