Position Overview Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health assignment covering the Berkeley / Oakland area . This is a field-based role working directly with patients in their homes, focusing on improving functional mobility and independence. Key Responsibilities Perform in-home patient evaluations and develop treatment plans Deliver skilled physical therapy services in a home health setting Complete accurate and timely OASIS documentation Monitor patient progress and adjust care plans accordingly Educate patients and caregivers on exercises and safety Maintain strong communication with interdisciplinary team Requirements Active California PT License (or pending with application) Minimum 1 year of Home Health experience REQUIRED OASIS experience REQUIRED BLS Certification (AHA) Valid driver's license and reliable transportation Assignment Details Schedule options: Tues–Sat or Wed–Sun 25 visits = 40 hours guaranteed Mileage reimbursed (IRS standard) Territory: Berkeley / Oakland area Locals accepted
