This role works closely with physicians and interdisciplinary care teams to deliver specialized wound care and ostomy management for patients in a rehabilitation setting. Key Responsibilities Provide expert care and management for patients with wounds, ostomies, and related conditions. Collaborate with physicians and interdisciplinary healthcare teams to develop and implement treatment plans. Educate patients and caregivers on wound management, prevention strategies, and recovery plans. Monitor patient progress and adjust care plans as needed to optimize outcomes. Ensure consistent adherence to clinical standards and best practices in wound care.
