Travel Physical Therapist (PT) – Home Health San Jose, California Facility: Sutter Health Care at Home Contract Duration: 13 Weeks (March 23, 2026 – June 22, 2026) Estimated Weekly Pay: $2,718.99 Schedule: Day Shift | 5x8 Hours (8:00 AM – 5:00 PM) Coverage Area: San Jose / South Bay / Santa Clara County Position Overview Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) to join the home health team at Sutter Health Care at Home in San Jose, CA. This role involves providing rehabilitation services to patients in their homes throughout the South Bay and Santa Clara County region. The Physical Therapist will evaluate patients, create individualized treatment plans, and provide therapeutic interventions aimed at improving mobility, independence, and overall quality of life. The ideal candidate will have prior home health experience and strong documentation skills. Key Responsibilities Conduct comprehensive in-home patient evaluations and reassessments Develop individualized physical therapy treatment plans Implement therapeutic exercise programs and mobility training Monitor patient progress and adjust care plans as needed Educate patients and caregivers on home exercise programs and safety practices Maintain timely and accurate documentation within the EMR system Coordinate care with physicians, nurses, and interdisciplinary healthcare teams Required Qualifications Minimum 2 years of Physical Therapy experience Home Health experience required Active California Physical Therapist License (in hand at submission) BLS Certification (AHA or American Red Cross) Preferred Qualifications Experience with Epic EMR system Strong independent clinical decision-making skills Experience managing home-based patient care Compliance Requirements Candidates must provide the following at the time of submission: Active California PT License BLS Certification (AHA or American Red Cross) Copy of all certifications Sutter Cover Sheet Two professional references within the last year Nursys verification within the last two weeks 7-year work history verification Additional Requirements Must be able to complete lab work within 3–5 days of signing the contract Must disclose any background issues or DUIs prior to submission Travelers must meet the 50-mile travel requirement (locals accepted at local rate) Orientation & Training Orientation modules completed during onboarding Modules are non-billable (1–5 hours) and included in non-billable orientation time Additional module time exceeding 16 hours may be billable Scheduling & Time-Off Policy Only one holiday request may be approved during the assignment Requested time off must be disclosed at the time of submission
