Care Coordinator

40 hours per week Job Title: Care Coordinator Company: Faithful Therapy TX Contract Details: Full Time, 40 hours per week Faithful Therapy TX is a leading mental health organization dedicated to providing quality care to individuals and families. We are currently seeking a dedicated and compassionate Care Coordinator to join our team on a full-time basis. As a Care Coordinator, you will be responsible for coordinating and managing the care of our clients, ensuring they receive the best possible treatment and support. Your role will involve working closely with our team of therapists and other healthcare professionals to develop and implement treatment plans for our clients. Key Responsibilities: - Client Communication: Answer calls, texts, and emails; book, rebook, and reschedule clients; manage no-shows and cancellations. - Lead Management: Contact new leads, move leads through CRM, and maintain accurate records. - Therapist & RDN Coordination: Ensure provider availability, relay client needs, and manage schedule adjustments. - Insurance Verification: Check client insurance coverage, document benefits, and update/manage CRM records. - Outreach & Sales: Promote and upsell counseling services; gather feedback and support retention. - Communicate with clients, families, and healthcare providers to ensure all parties are informed and involved in the care process - Maintain accurate and up-to-date client records and documentation - Update team on client progress and develop treatment strategies - Ensure compliance with all state and federal regulations and ethical standards Qualifications: - Completing or completed Bachelor's degree in social work, psychology, or a related field - Minimum of 2 years of experience in a mental health setting, preferably as a care coordinator or case manager - Excellent communication and interpersonal skills - Strong organizational and time-management abilities - Knowledge of mental health diagnoses, treatment options, and community resources - Ability to work independently and as part of a team - Proficient in electronic medical record systems and insurance verification - Must pass a background check We offer a competitive salary and benefits package, including health insurance, paid time off, and opportunities for professional development.
