Physical Therapist

Our client is seeking a Full-Time Physical Therapist for their Harrisburg territory. This role operates on a high-yield Pay Per Visit (PPV) model, offering some of the most competitive rates in the Susquehanna Valley. As a Home Health PT, you will be the clinical lead for patients recovering in their own homes. This is a role for a therapist who enjoys autonomy, values one-on-one patient interaction, and wants to see the direct impact of their interventions on a patient's daily life and safety. Key Responsibilities Clinical Assessments: Perform comprehensive OASIS evaluations to determine the patients functional baseline and establish a rehabilitative plan of care. Therapeutic Interventions: Provide skilled physical therapy, including gait training, strengthening, and balance exercises tailored to the home environment. Patient Safety: Conduct home safety assessments to identify fall risks and recommend necessary modifications or assistive devices. Interdisciplinary Care: Collaborate with Physicians, RNs, and Occupational Therapists to ensure a coordinated approach to the patient's recovery goals. Efficient Documentation: Utilize a mobile EMR system to document patient progress accurately and timely at the point of care. Qualifications Licensure: Active, unencumbered Physical Therapist (PT) license in the state of Pennsylvania. Education: Graduate of an accredited Physical Therapy program. Experience: Minimum of 1 year of clinical experience required; Home Health or Geriatric experience is a significant plus. Compliance: Active BLS certification and a valid PA driver's license with a clean driving record. Skills: Excellent communication skills and the ability to work independently in a field-based setting.
