Home Health Physical Therapist

·     Requirements:

o   Must have Ohio Physical Therapist License

o   Must meet productivity expectation of 30 points per week

o   Home health experience preferred

·        Job Description:

o   Patient Assessment: Conducts thorough evaluations to determine the patient's needs and goals.

o   Treatment Plan Development: Creates individualized therapy plans, often in consultation with physicians.

o   Treatment Implementation: Provides hands-on therapy, exercises, and education to patients.

o   Documentation: Maintains accurate records of patient progress and interventions.

o   Communication: Communicates with physicians and other healthcare team members to ensure coordinated care.

o   Patient and Caregiver Education: Provides education and counseling to patients and their families about their condition and treatment.

o   Supervision: May supervise physical therapist assistants, in accordance with applicable regulations, according to Indeed.

o   Progress Monitoring: Regularly evaluates patient progress and adjusts treatment plans as needed

·        Additional information:

o   13 week contract with possible extensions
