North Bend Medical Center is actively seeking Clinical Social Worker Associates (CSWA) or Licensed Professional Counselor Associates (LPCA) to join our team. This full-time role involves working within the integrated behavioral health model in our Internal and Family Medicine Departments.

Our approach differs from traditional mental healthcare—it’s a collaborative model that blends behavioral health with medical treatment, requiring proficiency in managing a diverse range of medical and mental health issues. As a Behavioral Health Consultant, you will be providing high-volume services through a population-based perspective that is accessible and routinely incorporated into primary care.

Collaborate with Primary Care Providers and staff to conduct brief, focused assessments and interventions targeting functional improvement for various conditions such as mental health challenges and behavior-related issues like stress management, sleep disturbances, smoking cessation programs for vaping or tobacco users, diabetes management support, chronic pain strategies etc. This dynamic position offers an exciting pace that's both challenging and fulfilling.

Key Responsibilities:
- Conduct evidence-based assessments to assist PCPs in recognizing and addressing mental/behavioral health concerns.
- Engage in short-term consultations with patients/caregivers focusing on resilience.
- Screen for psychosocial stressors using validated tools; collaborate on treatment planning.
- Utilize motivational interviewing techniques; provide psychoeducation on risk reduction/prevention.
- Implement cognitive-behavioral strategies to foster positive thinking/self-efficacy.
- Develop self-care plans emphasizing disease self-management skills.

- Master’s or Doctoral degree in Mental Health field
- Valid CSWA or LPCA certification/license

- Schedule: Monday – Friday, 8:00 am - 5:00 pm
