Registered Nurse - Hospice $2,500 Sign-On Bonus Available

We are currently seeking a Hospice Registered Nurse (RN) – Field Case Manager to join our hospice team.

POSITION OVERVIEW

The Hospice RN Field Case Manager provides skilled nursing care to hospice patients in home and facility settings.

This role requires strong clinical judgment, excellent documentation, and the ability to guide patients and families with clarity, empathy, and professionalism.

KEY RESPONSIBILITIES 

• Deliver comprehensive hospice nursing care in accordance with physician orders

  and hospice standards

• Perform patient assessments and develop, implement, and update individualized plans of care

• Manage pain and complex symptoms related to terminal illness

• Ensure accurate, timely documentation that meets Medicare and reimbursement guidelines

• Coordinate patient care with caregivers, physicians, and interdisciplinary team members

• Communicate effectively with patients and families, building trust and rapport at all levels

• Participate in Interdisciplinary Team (IDT) meetings and quality reviews

• Provide on-call coverage a minimum of one week per month, including evenings, weekends, and holidays as assigned

• Travel as required based on agency needs

REQUIRED QUALIFICATIONS

• Active Registered Nurse (RN) license in Pennsylvania

• Minimum 2–3 years of clinical nursing experience

• Hospice or palliative care experience strongly preferred

• Strong clinical assessment and critical-thinking skills

• Excellent communication with patients, families, and care teams

• Valid driver’s license and reliable transportation

PREFERRED QUALIFICATIONS

• Experience with Medicare hospice documentation

• Home health or hospice case management background
